Pediatric Care

Care-Plus nursing services exist to provide skilled evidence-based care to children with complex or additional needs in the community and all children who attend a special school. This provides support for the parents, carers, and school staff in meeting the health needs of children with complex needs.

The service is provided by a team of highly skilled staff made up of senior nurses, staff nurses, and health care support workers.
We work in partnership with other agency’s statutory, private, and voluntary sectors in order to meet the complex health needs of children and their families within the Trust’s footprint.

We’re experts in nurse-led, personalized at home, supporting those aged 0-25 with complex care needs in Greater London and surrounding counties. Our specialist care means children and young people can live within the family home and in their local communities without compromising high-quality clinical care packages.

Our approach

We work collaboratively with our clients and their families to develop a thorough care and support plan that accurately reflects a person’s holistic needs.  We also adopt a partnership approach to working with local authority partners, NHS services, multidisciplinary teams, therapists, and other community bodies involved in a person’s care.  This means we are well-informed so we can provide continuity of care, ensuring the right care is in place at the right time whilst responding efficiently to changing needs and strengthening care provision when required.

We work in partnership with other healthcare professionals, multidisciplinary teams, commissioning groups, and other therapy providers involved in our client’s care to ensure an integrated and holistic approach to the pediatric care we provide.

This holistic approach means our care is consistent and can adapt as your child grows and develops.

At Care-Plus, we know that having a child with complex needs can impact the whole family, so family involvement is central to our approach to providing pediatric care at home.  Our care teams will ensure you receive high levels of support as and when you need it.  The level of care provided is planned in partnership with everyone involved in a client’s care as we know through our experience, this ensures goals are reached, and outcomes are improved.  Our family-focused service allows children and young adults to enjoy life at home where they should be, participating in everyday family life.  We know through our experience that welcoming new carers into your home is not always easy, so we ensure we introduce all team members to you before they begin working in your home.

COMPETENT AND COMPASSIONATE PAEDIATRIC CARERS

When you choose Care-Plus  Nursing & Care, you can be reassured that the carer who cares for you or your loved one is experienced and expertly trained to provide the support you need to the high standards you should expect.

If you are receiving complex care at home, the carers we choose to support your needs will have at least six months of care or health sector experience before they come to care for you. In addition to completing our induction training program, which goes above mandatory requirements, they are trained by our registered nurses, clinical experts, and other healthcare professionals involved in providing care to a specific client.  This hands-on nurse training means our carers can competently undertake a range of clinical interventions.

We adopt a bespoke approach to training that ensures our carers have the knowledge, skills, and aptitude to deliver high-quality complex care that meets the very specific needs of our clients.
