Learning Disability

A learning disability affects the way a person learns new things in any area of life, not just at schoolIt affects the way they understand information and how they communicate. Around 1.5 million people in the UK have one.

This means they can have difficulty: understanding new or complex information, learning new skills, and coping independently A learning disability can be mild, moderate or severe, profound, or even multiple. CarePlus Health Services supports people with learning disability and those with multiple and complex needs to live their lives as they choose.

Person Centred Planning

To achieve these outcomes, we work with each person to help them develop a Person Centred Plan. We work with the person’s wider circle, their families, friends, advocates and other professionals to ensure the plan truly reflects their needs and preferences. The plan is outcomes focused and includes details of all areas of support required to meet each person’s individual needs, wishes and aspirations.

We ensure support plans are reviewed regularly so that they continue to be appropriate and effective. This is done with the involvement of the Service User, their families and other professionals. The Plan will be monitored to ensure that individual goals are being met and new goals being set to ensure people are being helped to reach their potential and are accessing new opportunities.

As we recognise that everyone is an individual, with needs and wishes that are different to the next person. We are able to provide care and support in a flexible way, from just a few hours each week to 24 hour support each day.
